摘要

目的:探讨帕金森病( Parkinson's disease,PD)患者视网膜黄斑区神经节细胞层结构变化的规律,为 PD 的早期诊断和病情评估提供指导。

方法:收集在福建省泉州市第一医院就诊的原发性 PD 患者 45 例(PD 组),收集与 PD 组年龄相匹配的健康志愿者30例(对照组)。所有研究对象均采用光学相干断层扫描(opticalcoherence tomography,OCT)对双眼黄斑区神经节细胞层厚度和体积进行测量,统计分析其在 PD 组与对照组之间的差异。应用 Pearson 相关分析方法对 PD 患者的神经节细胞层参数与其 Hoehn-Yahr(H-Y)分级和运动障碍学会统- PD 评定量表 Ⅲ(Movement DisorderSociety Unified-Parkinson Disease Rating Scale Ⅲ ,MDS-UPDRS Ⅲ )评分进行相关性分析。

结果:与对照组相比,PD 组患者的黄斑区神经节细胞层厚度变薄(P<0.05),体积变小(P<0.05),并且均与 PD 组患者的 MDS-UPDRS Ⅲ评分和 H-Y 分级呈负相关。

结论:PD 患者的黄斑区神经节细胞层厚度明显变薄、体积变小,并且随着病情的加重,其厚度和体积的变化也更明显。0CT检测下黄斑区神经节细胞层厚度及体积或可作为PD的-个生物标志物,为疾病的早期诊断及病情评估提供一定的帮助。

Abstract

Objective: To explore the regularity of the structural changes of the ganglion cell layer inthe retinal macular region of Parkinson's disease (PD), and to provide guidance for theearly diagnosis and condition assessment of PD.

Methods: 45 patients with primary PD (PD group) and 30 healthy volunteers who matchedthe age of PD group (control group) were enrolled from Quanzhou First Hospital, FujianAll subjects used optical coherence tomography (OcT) to measure the thickness and volume of ganglion cell layer, and statistically analyzed the differences between PD group andcontrol group. Pearson correlation analysis method was used to analyze the correlationbetween ganglion cell layer parameters and Movement Disorder Society Unifed-ParkinsonDisease Rating Scale I (MDS-UPDRS Il)score and Hoehn-Yahr (H-Y) grade in PDpatients.

Results: Compared with the control group, the ganglion cell layer in the macular area ofPD group were thinner (P<0.05)and smaller (P<0.05). The thickness and volume ofganglion cell layer in PD group were negatively correlated with the MDS-UPDRS Il score and H-Y grade.

Conclusion: The ganglion cell layer in the macular region of patients with PD was thinnerand smaller, and the thickness and volume change more obviously with the aggravation of the disease. The thickness and volume of the ganglion cell layer in the macular area measured by OCT may be used as a biomarker of PD to provide some help for the early diagnosis and evaluation of the disease.
